Making an appointment
To schedule an appointment with one of our physicians, please call our office directly at (479) 521-3300 between the hours of 8:30 a.m. and 5:00 p.m. Monday through Friday. Each of our physicians is scheduled in the office at certain times during the week. While you are on the telephone, our staff will advise you of these times in order to make the most convenient appointment for you.
We will make every effort to provide an appointment with the surgeon you or your doctor request. If no preference is indicated, an appointment is made at the first available time that is convenient for you. Subsequent appointments are made with the same doctor to provide the best possible continuity of care.
Frequently our doctors will need to view any lab, x-ray, or prior testing that your family physician has sent you for. Please ask the receptionist when you are making your appointment if you will need to bring these items with you when you come.
It will be necessary to complete some paperwork on your first visit to our office. For your convenience, this can be done in advance of your visit. Click HERE to obtain copies of these forms that you can print and fill out before your appointment.
