Insurance and Health Plans

In this age of managed care, it is hard for us to keep up with the different rules of individual insurance plans; therefore, it is the patient’s responsibility to:

  1. Verify that the physician you will see is “in network” or on your plan.
  2. Obtain any necessary referrals from your primary care physician.
  3. Pay any co-pay or co-insurance required by your insurance at the time of your visit.

If you have insurance coverage that requires an annual deductible be met, and you have not met that deductible, you will be required to pay the allowable amount in full at the time of your visit. if you have met your annual deductible for the year, you will need to pay your co-insurance percentage which may vary according to insurance plan. This amount will be determined at the time of your visit.

As a courtesy, we will file your insurance requesting that all payment come directly to our office. If we have difficulty getting your insurance company to pay, we request that the patient assist our office in obtaining resolution of the bill or the balance may be turned over to the patient’s responsibility to resolve the balance on the bill.

SELF-PAY patients are asked to pay in full at the time services are rendered. Any exception to this policy must be approved at the time your appointment is made. For your convenience, we accept MasterCard, Visa, cash, and personal checks. A discount will be given to self-pay patients who pay their bill in full at the time of service.

During your visit our insurance coordinator will call your insurance to verify your eligibility and benefits. If you are scheduled for surgery, she will meet with you to go over the estimated portion you will be responsible to pay. You are encouraged to pay this amount in full or set up payment arrangements before you leave the office.

Our surgeons participate in the Medicare program. This means we will accept Medicare’s allowable payment and you will be responsible for the 20% co-insurance amount and any deductible that applies. You will be asked to pay your 20% co-insurance on the day of your visit.

If you are unable to find the carrier on our list that appears on your card, please call the customer service number on the back of your insurance card to verify that the physician you will see is in-network.
